ZUNG SELF-RATING DEPRESSION SCALE
A simple set of questions to help understand your level of depression.

Instructions:
Read each statement carefully. Use the following scale to indicate how often you have felt that way during the past two weeks. (If you are on a diet, respond to statements 5 and 7 as though you were not on a diet.)

1 = none or a little of the time
2 = some of the time
3 = good part of the time
4 = most or all of the time
5 = all the time

_______ 2. Morning is when I feel the best.
_______ 3. I have crying spells or feel like it.
_______ 4. I have trouble sleeping through the night.
_______ 5. I eat as much as I used to.
_______ 6. I enjoy looking at, talking to and being with attractive women/ men.
_______ 7. I notice that I am losing weight.
_______ 8. I have trouble with constipation.
_______ 9. My heart beats faster than usual.
_______ 10. I get tired for no reason.
_______ 11. My mind is as clear as it used to be.
_______ 12. I find it easy to do the things I used to do.
_______ 13. I am restless and can’t keep still.
_______ 14. I feel hopeful about the future.
_______ 15. I am more irritable than usual.
_______ 16. I find it easy to make decisions.
_______ 17. I feel that I am useful and needed.
_______ 18. My life is pretty full.
_______ 19. I feel that others would be better off if I were dead.
_______ 20. I still enjoy the things I used to do.

SCORING:
Reverse responses to items 2, 5, 6, 11, 12, 14, 16, 17, 18 and 20 (that is, 1 =5, 2 = 4, 3 = 3, 4 = 2, 5 = 1).
Then add all the numbers to obtain a total score, which can range from 20 to 80. Scores from 50 to 59 suggest mild to moderate depression, from 60 to 69 indicate moderate to severe depression and 70 and above indicate severe depression.
